Connections Housing for the homeless in Baltimore
Home Connections provides immediate access to permanent housing, intensive case management and other supportive services to 60 chronically homeless individuals in Baltimore, Maryland. Residents also have access to employment assessment and referral, service linkage, health assessment and referral, support groups and individual peer support, life skills development, and recreation.
Home Connections uses a "Housing First" approach that is based on the premise that re-housing should be the central goal of working with the homeless. By providing housing first, followed by supportive services, participants receive the immediate stability they need so that they are prepared to engage in services that address the reasons for their homelessness. Participants are expected to maintain their units in a clean and orderly manner, be good neighbors, and pay a portion of the rent based on their income level. Program staff visit participants in their home at a minimum of once per week to ensure that they are maintaining their housing and level of independence, while also assisting them in accessing the treatment and supportive services they need.
