Connections provides 28 chronically homeless individuals
with immediate access to permanent housing, as well
as intensive case management and other supportive services.
Residents live in private homes scattered throughout
the community located in neighborhoods that match the
needs of each individual.
With staff offices at Beans & Bread Center, Home
Connections uses a "Housing First" approach
that is based on the premise that re-housing should
be the central goal of working with the homeless. By
providing housing first, followed by supportive services,
participants receive the immediate stability they need
so that they are prepared to engage in services that
address the reasons for their homelessness.
Participants are expected to maintain their units in
a clean and orderly manner, be good neighbors, and pay
a portion of the rent based on their income level. Program
staff visit participants in their home at a minimum
of once per week to ensure that they are managing well,
while also assisting them in receiving the treatment
and services they need.
